Designing for Adherence: What Really Keeps Participants Engaged?

In decentralized, digital, and consumer-facing trials, the participant is more than just a data source. They’re a user of an experience, a decision-maker, and – most importantly – a human navigating daily life. So, what really drives study adherence? And how can researchers design protocols that participants actually want to follow?

Adherence is the silent determinant of success in many clinical and consumer health studies. Regardless of how sophisticated a study design is, or how powerful its endpoints, none of it matters if participants don’t show up, don’t comply, or drop out entirely. And yet, adherence remains one of the most overlooked challenges in research – especially outside traditional clinical settings.

1. Clear Value Proposition for the Participant

If you want someone to adhere to your study, they need to understand why they’re doing it. This is true in pharma trials , but even more so in consumer wellness research, where stakes may feel lower to the participant.

Design tip: Incorporate short-term, visible wins into your protocol. Even a simple weekly check-in showing progress can re-engage a drifting participant.

2. Frictionless Experience Design

Many dropouts aren’t due to disinterest, but to difficulty. Confusing instructions, clunky apps, forgotten surveys, or unclear expectations can lead to gradual disengagement.

Researchers need to think like UX designers. Ask:

Modern participants, especially in digital trials, expect the same seamless experience they get from fitness apps or food delivery. Study software should meet that bar.
